GAIR will be available for the use of pilots of aircraft ranging from the traditional lighting right through to jumbos. The objective of the system is to ensure aircrew confidentiality (unless an A('IR has deliberately been breached) and to build up an incident database that will hopefully serve to enhance air safety at all levels in the years to come

If you’re like most pilots, you probably see a ‘two-two-five’ as the ultimate punishment for wayward pilots. The reality is that Form 225 is simply the official departmental incident report form.
But, it’s never been a successful way for the Department (BASI remains with the DoTC, not the CAA) to find out what’s really going on in the real world of flying. Bureau of Air Safety Investigation research has shown that human factors figure in about 75% of accidents but only 12% of incidents, with GA contributing 9% and RPT the remaining 3%.
Interviews with pilots have confirmed suspicions that the real figure is much higher, but they’re reluctant to put in reports on their own mistakes for four basic reasons:
